Care Manager will begin with a very comprehensive and in-depth
assessment of physical, mental, social, emotional, and financial
strengths and weaknesses, and functional abilities. The
CONNECTIONS difference is found in
 our
unique capabilities, exceptional knowledge, and the ease with which
we get acquainted with our older clients and make them feel
comfortable and at ease. Based on the results, a care plan is
developed. We review our recommendations with you and discuss
the best available options. You may take over from there, or
you may wish to have the Care Manager implement and coordinate the
services and do ongoing monitoring. The Care Manager can
handle the personal care needs and can even take care of home
maintenance needs if necessary. Our
service includes regular home visits and telephone calls to track
and respond to any changing needs. We provide emotional
support when someone is afraid or lonely. Everything is
discrete and confidential. Pampering is our specialty!
You get a single point of contact for all the needs of your loved
one. It is simple, personal, private, and cost effective.

Reasonable and
Affordable Rates
There is no
charge for the initial inquiry by phone. There is a fee for
assessments, consultations, and on-going services at a reasonable
hourly rate billed in 10-minute increments for the time spent
working with or for you.
